Company Policies: Payments, Refunds, and Cancellations


An initial consultation is required for all patients, $100 with Nurse Practitioners, $200 with Physicians. $100 of the consultation fee will be applied toward your first treatment.

Appointments / Cancellations / Late Arrivals

All appointments require a credit card number to be kept on file. All cancellations require 48 hours notice. Failure to cancel within 24 hours of your scheduled appointment will be considered a “Late Cancel/No Show” and the following fee structure will be applied to the credit card on file. Fee structure: $50 for every 30 minutes booked on the schedule for Medical providers and $30 for Medical Estheticians.

If you are more than 15 minutes late for your appointment, we reserve the right to reschedule your appointment or cut your scheduled service short. This may result in a charge for services rendered or a fee based on our no show/late cancellation policy.


Payments are required in full at the time of each treatment. We accept Visa, MasterCard, Discover, Debit Cards, cashiers check, cash, and gift certificates. Financing is available but will carry a 3% administrative fee for processing.

Insufficient Funds: A 20% charge will be applied to any unpaid balances for every 30 days past due. We reserve the right to apply this charge to any credit card that you have on file with Arviv Medical Aesthetics, without notice. All patients are required to keep a valid credit card number on file.

Balances due to insufficient funds as well as due to Late Cancel/No Show fees will remain on your account and a 20% charge will be applied to any unpaid balances for every 30 days past due . No further appointments will be scheduled until the balance is paid in full.

Packages: To receive package pricing, payment must be made at the time of the first treatment or sale. Package pricing is nonrefundable, nonreturnable, and non-transferable and may not be applied to other treatment areas. Package expiration dates are per the fine print on date of purchase. All packages must be redeemed within 12 months from payment. Unused services by the date of expiration will be forfeited. If you are unable to complete a package, services completed will be calculated at full price and the remaining may be issued as a credit towards another service.

Disputes: We ask that if you have a complaint please contact a manager to resolve it. If you dispute any payments for services rendered, regardless if you are refunded by your credit card company, we reserve the right to pursue collections. In addition, you will be responsible for any legal fees associated with the collection process.

Gift Cards

Gift Cards, including electronic and mobile-Gift Cards, are available in any denomination. *Complimentary Gift Cards may only be used toward full-price services.

Medical Changes

Please notify us with any medical or health changes at the time of each appointment, so we can safely treat you.


Skin care products and services are non-refundable/non-returnable.
